Initial Forms

Please send either Kerry or Banjo an email if you’re a prospective client and we will forward you a copy of the initial paperwork. The info is useful for treatment planning, your informed consent, billing insurance, and clarifying office policies.

In-network insurance plans:

Always check with your insurer, as some programs have multiple plans that reimburse out of network providers (often at a different rate),  and some have multiple plans that may or may not contract with me.  Finally, ask about Copay and Deductible information.  Some insurance programs have a lofty deductible that will require you pay before they start paying.

Cancellation Policy:

A 48-hour notice is needed if you are unable to keep your appointment. 
Clients will be billed for all appointments missed without 48 hour notice.
